If you are a new patient, please visit our New Patient section for information on your upcoming appointment.

In preparation for your initial appointment, please download and print the Patient Intake Consent Form and Patient Body Diagram, complete both, and bring them to your appointment.

If you are an established patient with a new condition, you will be asked to complete an updated medical history form upon check-in for your appointment in addition to the above-mentioned forms

If you are being treated for an auto-related injury, please also download, print, and complete the Patient Auto Form. You will also need to bring your auto insurance card, claim information, and the date of the accident. Patients being seen for auto-related injuries should be printing all three of the forms in the Patient Forms section.

Please arrive 10-15 minutes early to your appointment so you will have time to fill out these forms if you have not done so already.

Please also bring your photo ID and health insurance card(s) to all your appointment.
